It is a little unrealistic. Kids with limitations due to health concerns are generally encourage by their health professionals to try to live as normal a life as possible so it is unlikely that these kids would be so restricted in what they could do that they could only have other friends with health issues. If their health was that compromised then making new friends would be a secondary concern next to finding an effective treatment and just surviving another day. While these kinds of friendships would not be actively discouraged because it is important for these kids to have people around them who can understand their struggles hospital resources would not be utilized to specifically arrange them.

Most children with epilepsy live lives that to the outside observer are hardly different from any other kid. Other than being careful about activities were falls from heights could happen or getting extremely hot or cold they can do almost anything. Kids with heart problems, unless they are receiving a transplant or are in the hospital to adjust to a new regime of medication (usually a matter or days) are usually managed as outpatients and other than restrictions on active sports that could lead to overexertion they can do many activities normal kids do like playing video games, going to the mall, etc.

If you could find a different setting or circumstances for them to meet then the story could be interesting if handled properly. It's been done before but if you can find a way to make it fresh then go for it.

Cheers.
References :
Good idea. Both are real life characters. By dramatizing the attitude of people and kids around them based on their kindness, meanness, sympathy etc and their problems of their parents a good plot and story would emerge.
References :
I believe it sounds very realistic and would be of great interest to many. I know any sort of valve problem affects the heart. Just a thought, good luck!!
References :
I would change the part where they meet in the hospital, but no, I like it :]
You could just say they have a heart murmur
